Survey Help?

Hello all!

I am taking a course this term that requires us to informally survey individuals on any topic of our choosing. I'm sure it's not too big of a surprise to most that I am selecting to survey individuals in the United States on their consumer product and health knowledge/opinions. To make it easy, I created a survey online via Survey Monkey. Since it's free I could only create 10 questions, so the survey should be quick and painless....relatively easy to complete. AND the responses are voluntary and anonymous with no real risks involved (except maybe increased education?!).

The survey will be open until 8pm on this Sunday, November 23rd. PLEASE feel free to pass the link on to other individuals who you think would take the time to help me out,(especially since many of you close friends and family already will skew the data based on the things I've blogged and talked with you about during the last couple of years AND it would be nice to have a wide cross representation). However if you are also willing to help me out by taking it yourself, I'd greatly appreciate it! Just answer truthfully and to the best of your ability. It doesn't matter if it's right or wrong, as the point of the gathered data is to design a (fake) media campaign to educate the public on the topic anyway.

You can always comment with feedback or if you want to know the answers to some of the questions. After Sunday, I'll post the results of the quiz and the answers to the questions. Thanks in advance to those of you who are willing and/or able to help me!
